Outpatient Referrals from Providers, Agencies, and Institutions

Wholeness 2 Peace Care Services accepts outpatient treatment referrals from providers, agencies, courts, schools, and community partners throughout Maricopa County.

We coordinate care promptly and maintain clear communication with referral sources to ensure continuity and accountability.

We serve youth and adults. Psychiatric medication management is available for ages 8 and up. Appointments are available in our Phoenix office or by video.

Services Available for Referral

Outpatient Counseling

Structured therapeutic services for youth and adults requiring clinical intervention. Services include individual and family therapy, treatment planning, and coordination with external providers when appropriate.

Medication Management

Psychiatric evaluation and ongoing medication management provided within our licensed outpatient setting. Referrals may include clients requiring medication stabilization, psychiatric consultation, or collaborative care alongside therapy services.

Direct Support (BHT Services)

One on one behavioral health technician support delivered under clinical supervision. BHTs work with clients in the home, at school, and in the community on daily routines, coping skills, behavioral stabilization, and goal-based progress. Available for youth and adults.

Case Management

Support with care coordination, community resource linkage, service navigation, and stabilization planning for clients requiring additional structured support.

Comprehensive Intake & Clinical Assessment

Structured diagnostic assessment and treatment planning for youth and adults entering outpatient services. This includes evaluation of presenting concerns, functional impairment, risk factors, and level of care determination.

DUI Screening, Treatment, and Education

Court-related DUI screening, education, and treatment services delivered in accordance with state requirements. Coordination available with referring legal entities when applicable.

What to Include With Referrals

  • Client full name and date of birth
  • Contact information for the client or guardian
  • Insurance information, including plan and member ID
  • Reason for the referral
  • Any supporting documents, such as prior assessments or court orders

Send us the client’s full name and date of birth, contact information, insurance information, the reason for the referral, and any supporting documents.

What happens after you send a referral

  1. We confirm we received it within one business day.
  2. We contact your client to schedule.
  3. Once insurance is verified, we schedule the first appointment, usually within a week.
  4. We let you know when they are scheduled, and we tell you if we cannot reach them.
